Open Question: What makes a person stand out when they're applying to college?

I'm a rising sophomore and I want to be a doctor.I got a 4.0 GPA freshman year but I did not do many extracurriculars. I'm thinking of going to Georgetown or George Washington University. I am going to do more extracurriculars and sports next year. I also volunteer at a nursing home and the hospital. What are some unique things I can do to impress colleges?
